Active vs inactive — what it means
- B12: methylcobalamin is the body's active transfer form.
- Folate: L-5-MTHF-Ca is the circulating form your cells actually use.
- B6: P5P (pyridoxal 5′-phosphate) is the phosphate your enzymes reach for.
Inactive forms ask your body to do the conversion. For many people that conversion goes fine. For some, it does not — and when appetite is down for weeks, the cost of a slow conversion shows up as "I am doing everything right and still wiped."
The homocysteine angle
B12, folate and B6 sit on the homocysteine pathway. When any of them is low or poorly converted, homocysteine drifts high. Bringing the active forms together at modest doses — not mega-doses — is how a daily habit helps the pathway without turning a capsule into a dare.
The MTHFR footnote, plainly
Some people carry common variants in the MTHFR pathway that make folic acid → methylfolate less efficient. For them, L-5-MTHF bypasses the step that can be slow. That does not mean everyone needs it; it means for the slice that does, folic acid can be a roundabout route.

FAQ
Should I keep my old B complex too? Probably not — add the totals. Two B complexes at once can put folate and B12 well above the useful range.
Is methylated always better? For people who convert well, plain forms work. Active forms are a deliberate hedge so the people who convert slowly are not left waiting on their own chemistry.
Any interaction to watch? High folate can hide a B12 deficiency signal on labs. Tell your clinician what you take, especially if appetite has been low.
