What to eat on Wegovy to avoid muscle loss

A practical guide to protein, resistance training and the nutrients that get squeezed when you eat less on semaglutide, with amounts and sources.

To protect muscle on Wegovy, eat 30 to 40 grams of protein at each of three or four meals, which is roughly 1.2 to 1.6 grams per kilogram of body weight a day, and lift something heavy two or three times a week. Eat the protein part of the plate first, while appetite is strongest, and let the rest go.

That range is a target, not a rule, and it is not right for everyone. If you have kidney disease, are pregnant, or are on other medication, your doctor or dietitian should set the number with you before you change anything.

Why does muscle loss happen on Wegovy?

Any large weight loss takes some lean mass with the fat. This is true of dieting, of bariatric surgery, and of GLP-1 medication. In the STEP 1 trial of semaglutide 2.4 mg (Wilding et al., New England Journal of Medicine, 2021), a body composition substudy found that participants lost both fat mass and lean mass, with fat mass falling proportionally more, so body fat percentage went down overall. The catch is that lean mass still went down in absolute terms.

The 2025 joint advisory on nutrition during GLP-1 therapy, from the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association and The Obesity Society, puts numbers on the pattern: in STEP 1, about 38 percent of the weight lost was lean body mass, and modelling suggests roughly 10 to 25 percent of total weight lost is muscle when no structured strength training is in place. Two things drive it. You are eating a lot less, so protein intake drops along with everything else. And appetite suppression makes the food that does get eaten skew toward what is easy, which is rarely the protein.

So the job is narrow: keep protein up while total calories come down, and give your muscles a reason to stay. The same pattern holds for tirzepatide (Mounjaro, Zepbound) as for semaglutide (Wegovy, Ozempic). The drug names differ, the maths does not.

How much protein per day on Wegovy?

The RDA for protein is 0.8 g per kilogram of body weight, which is a floor for preventing deficiency in a person at stable weight. It was never designed for someone in an active deficit.

During weight loss, the evidence lands higher. The 2025 joint advisory notes that 1.2 to 1.6 g/kg of body weight per day has been proposed during energy restriction. A meta-analysis of protein supplementation during resistance training (Morton et al., British Journal of Sports Medicine, 2018) found that higher protein intakes support lean mass gains when paired with training, with benefits levelling off near 1.6 g/kg. And the International Society of Sports Nutrition position stand (Jager et al., 2017) recommends 1.4 to 2.0 g/kg for people who train regularly, and notes that spreading intake across the day beats loading it all into dinner.

For a 90 kg person, 1.4 g/kg is about 126 g of protein a day. On a medication that makes 1,200 calories feel like plenty, that takes planning.

Spreading it out matters more than usual here. Muscle protein synthesis responds to a meaningful dose of protein at a sitting, roughly 25 to 40 g depending on body size and age, rather than to the daily total arriving in one go. Four modest hits beat one enormous dinner you cannot finish anyway.

Which nutrients go quiet when you eat less?

Do you need to lift weights on Wegovy?

Eating protein without a training stimulus is like posting a letter with no address. Resistance training is the signal that tells the body which tissue is worth keeping when energy is short. Trials of weight loss with and without resistance exercise consistently show better lean mass retention in the training arm, and the Morton meta-analysis found the same pairing effect from the other direction: protein plus training builds and keeps more lean mass than either alone.

Two or three sessions a week is enough. Full body, compound movements, something that gets genuinely hard by the last few repetitions. Bands and bodyweight count. Walking is excellent for many things and is not a substitute for this one.

What should you eat when nothing sounds good?

The honest obstacle is not knowledge. It is that on dose escalation week, chicken can smell wrong from three rooms away.

Things that tend to survive a suppressed appetite:

  • Cold protein. Yoghurt, cottage cheese, kefir, tinned fish. Cold food carries less aroma, which helps.
  • Liquid protein. A shake goes down when a plate does not.
  • Salty over sweet. Broth, miso, savoury eggs.
  • Small and often. Six small things beats three meals you abandon halfway.

Fibre and fluid deserve a line of their own. Slower gastric emptying plus less food plus less water is a combination that makes constipation likely. Drink more than feels necessary and keep some vegetables and legumes in the rotation.

How much protein should I eat on Wegovy? Around 1.2 to 1.6 grams per kilogram of body weight per day, split across three or four eating occasions. A 75 kg person lands near 90 to 120 g. Confirm the number with your clinician if you have kidney disease or any condition that restricts protein.

Can protein shakes replace meals on a GLP-1? They can cover a meal you cannot face, and many people rely on one or two a day during dose escalation. Whole food still brings fibre, potassium and the rest, so treat shakes as the reliable backup rather than the whole plan.

Does taking a supplement stop muscle loss? No. Protein intake and resistance training are what preserve lean mass. Vitamins and minerals support normal muscle and energy function; they do not substitute for either.

Why am I so tired on Wegovy? Eating substantially less usually means less iron, less B12 and fewer calories overall, and fatigue is a common early complaint. Should I still lift if I only have twenty minutes? Yes. Two or three short full-body sessions a week beat none, and consistency matters far more than session length.

Does this change if I am on Mounjaro or Zepbound instead of Wegovy? No. The protein target, the per-meal split and the lifting advice are the same across semaglutide and tirzepatide. Both drug classes suppress appetite the same way, so the plate problem and the fix carry over unchanged.
