Insomnia on semaglutide: what to know
A warm, practical guide to insomnia on semaglutide, what the research does and does not show, and what to raise with your prescriber.
By the Knurle editorial team · How we source this
Some people report insomnia while taking semaglutide, but a 2024 records study in EClinicalMedicine did not find a higher 12-month risk of insomnia than with three other diabetes medicines. Sleep can still change for other reasons, including nausea, schedule shifts, or stress. Tell your prescriber if it persists.
We are not your clinician, and this page is not a diagnosis. If you are awake most nights, or the tiredness follows you into the day, bring it up with your prescriber rather than waiting for your next refill.
Can semaglutide cause insomnia?
The honest answer is that it looks uncommon as a direct effect, but it is not impossible for you.
A 2024 cohort study in EClinicalMedicine looked at adults with type 2 diabetes who were prescribed semaglutide and matched them with people taking sitagliptin, empagliflozin, or glipizide. Insomnia was one of 22 neurological and psychiatric outcomes tracked within one year. The study did not find semaglutide associated with a higher risk of those outcomes.
A 2026 study in Communications medicine compared adults with obesity who started tirzepatide or semaglutide with adults who started other prescription weight-loss medicines. Over 24 months, semaglutide was associated with lower hazards of anxiety and depression than naltrexone-bupropion in adults with and without type 2 diabetes. In adults without type 2 diabetes, tirzepatide was associated with higher hazards of anxiety and insomnia than semaglutide, with an insomnia hazard ratio of 1.13, 95% CI 1.05-1.21. That is a comparison between two medicines, not proof that semaglutide causes sleeplessness.
A 2026 chart review in the Journal of Psychiatric Research examined 226 adults prescribed GLP-1 receptor agonists at a private university hospital. Semaglutide was the most frequently prescribed medicine at 73%. Most patients with depression or anxiety remained stable. New-onset depression and anxiety were reported in 9.0% and 8.7% of affected patients, and insomnia also emerged in a subset. The authors say prospective studies are still needed to clarify causality.
Insomnia on semaglutide: what people report
Social media research captures the conversation, not a diagnosis.
A 2023 study in Brain sciences analyzed Reddit, YouTube, and TikTok posts about GLP-1 receptor agonists. Sleep-related issues, including insomnia, were the most common mental health matches, with 620 insomnia mentions. A 2025 observational study in JMIR Infodemiology reviewed Facebook posts from 2022 to 2024 and found insomnia mentioned in a cluster with anxiety, depression, fatigue, and restlessness.
Those studies show what people talk about online. They cannot tell you why your own nights changed.
Real-life causes often sit next to the medicine. Nausea, smaller meals, constipation, caffeine timing, schedule changes, and worry can all keep you awake. Our constipation guide and our guide to eating enough on GLP-1 medications cover the practical side. If you are up at night because your stomach is unhappy, that is a different fix than a racing mind.
What helps insomnia on semaglutide
Start with the boring things, because they are the ones you can control. Keep a regular wake time, get morning light, keep the bedroom cool and dark, and move the phone across the room if you scroll at 2 a.m. In Colorado, morning light is easy to find and free. The kettle can wait until after your caffeine cutoff; we know, the afternoon kettle is a personality.
Food and fluids matter too. If you are barely eating, aim for protein at each small meal and keep water in the fridge where you can see it. Our guide to eating enough on GLP-1 medications has simple ideas for low-appetite days.
When meals get smaller, nutrient questions follow. A 2026 chart review recorded average weight loss of 6.7% over a mean treatment duration of 20.2 months. The amounts below are not sleep doses; they depend on your labs, your medicines, and your diet.
| Nutrient | Typical daily amount | Why it comes up | What to eat for it |
|---|---|---|---|
| B12 | Ask your prescriber or pharmacist | B12 questions are common on GLP-1 medicines | Eggs, fish, meat, fortified foods |
| Magnesium | Ask your prescriber or pharmacist | People often ask about it when sleep changes | Pumpkin seeds, almonds, black beans, leafy greens |
| Iron | Ask your prescriber or pharmacist | Ask about testing before adding extra iron | Red meat, lentils, spinach, beans |
| Zinc | Ask your prescriber or pharmacist | Protein intake can dip when meals shrink | Oysters, beef, pumpkin seeds, cashews |
If you want a plain-language starting point, our B12 guide explains what is known and what is still unclear.
When to talk to your prescriber
Bring up insomnia if it happens most nights, lasts more than a couple of weeks, or arrives with low mood, anxiety, or heavy daytime sleepiness. Do not stop or change your dose on your own. Your prescriber can look at other causes and help you weigh sleep against your treatment goals.
If food is the bigger struggle, Keel is the thing we happen to make; it lives on our own bathroom shelf and contains digestive enzymes, probiotics, ginger and peppermint plus methylated B12, folate, B6, iron, zinc, magnesium, biotin, D3 and BioPerine, two capsules a day.
